We can easily add your logo, individual name and or department name to our Flu Shot Champions Trophy. The minimum order is 1 trophy. Add $ 10.00 to add a name and or department (per trophy). Add $ 20.00 to add name and or department plus company logo (per trophy). There is a one-time $75.00 set-up fee to add your logo. YOUR FIRST ORDER MUST BE PRE-PAID WITH A CREDIT CARD OR COMPANY CHECK.
On approved credit, future orders can be billed Net 15 Days with a written Purchase Order (this is required before we can ship your order). You can place an order online using a Puchase Order. Select "Purchase Order" as method of payment. And enter your P.O. number where indicated.
